Some clients have to make a co-payment for certain types of services. For others, all services are free. Some clients depending on income pay portion of monthly premium as well.
Application Process:
Can pick-up application at any welfare application office or the Med-Quest office. Screening interview-face-to-face. Written application. Appointment required. May phone to have application mailed. Need to bring Social Security card and proof of income.
Eligibility Requirements:
Must be a Hawaii resident and be a United States citizen or permanent legal alien. Must have entered the U.S. prior to August 22, 1996. Must be under 65 years old, not certified as blind or disabled, and not be living in a public institution. Must meet in
